The information documented on a certificate is taken from the original Registration of Birth, which is completed at the time of birth.

To order a certificate, you must be the person listed on the birth certificate or have one of the following relationships to that person:

• A parent
• A guardian, trustee or person with power of attorney (proof required)
• Any person declared as the Designated Agent
Any person with an order from the court (proof required)
• The legal representative (proof required)
• Any adult next of kin of a deceased person listed on the birth certificate
• A person adopting a child may apply for that child’s original birth certificate. (Proof required)

At Registries Direct you can apply for a Marriage Certificate with an application form and government issued photo ID. Wait at least two weeks from the date of marriage to order your certificate, this ensures that Vital Statistics has received the registration of marriage.

To apply for an Alberta Marriage Certificate, you must be:

• One of the married parties
• A person declared as the Designated Agent
• The legal representative for an eligible applicant (proof required)
Any person with an order from the court (proof required)
• A guardian, trustee or person with power of attorney for a person whose marriage is registered
• When a party to the marriage is deceased, their adult next of kin may apply
• An executor of an estate when either party to the marriage is deceased

At Registries Direct you can apply for a Marriage Certificate with an application form and government issued photo ID. Alberta Vital Statistics maintains a record of all deaths that occur in Alberta and can only issue certificates/ documents for events that occurred in Alberta. Death certificates are issued by using the information from the original Registration of Death, completed at the time of death.

You must be one of the following to obtain an Alberta death certificate:

• Any adult next of kin of the deceased person
• A guardian, trustee or person with power or attorney for the deceased person whose death is registered by court documents (proof required)
• Any person declared as the Designated Agent who is an adult next of kin to the deceased person
• The legal representative for an eligible applicant or a legal representative who requires it for official duties (proof required)
• The executor of the deceased person’s estate
• A funeral home representative who is making or had made arrangements for the deceased person

Before you get married in Alberta you must purchase a marriage licence.

Below are some requirements:

• Both parties to the marriage must apply together for their marriage licence.
• Both parties must provide valid identification.
• For licence to be valid you must be married within 90 days from the issuing of the marriage licence and must be married within the borders of Alberta .
• Applicants must be at least 18 years of age. (Persons under the age of 18 must obtain consent to marry)
• If the marital status of either party to the marriage is divorced copies' of the final court documentation is required.
• The bride and groom cannot be related.
• If either party is not fluent in English, they must provide a certified translator/interpreter who is over the age of 18 and is fluent in English as well as the language he/she is translating.

To legally change your name you must submit an application to Vital Statistics. Come into Registries Direct and pick up the application. Fingerprints must also be submitted. If the person whose name is being changed is under 18, the fingerprint form does not have to be done.

The applicant must be:

• 18 years of age or older
• A resident of Alberta

Applying to change one or more of the following:

• Their own name
• Current spouse's/adult interdependent partner's name • Their child's/ward's name

Not applying to change:

• A deceased person's name
• Their spouse's/adult interdependent
partner's child's name when the applicant is not the other parent, by birth or adoption, of the child or the applicant does not have custody/ guardianship of the child.
